I have installed Theater and created an album template for videos that is set to:presentation type=video, video source=include h.264....
When I try to add mp4 videos to the LR album I get the message "this publish service does not support video". How do I add MP4 videos?

Videos are not exported by Lr, but need to be uploaded to your site using FTP. But the recommendation is to serve videos from a sharing site such as Vimeo or Youtube.

Just because you can, does not necessarily mean that you should. For example, if you're on a shared hosting plan such as those offered by Bluehost, then HTML5 Video is probably not for you. Serving video directly from your site will chew through your bandwidth quickly.
